Membership fees scale by age: $35/month for children ages 6-17, $100/month for ages 18-24, $175/month for adults 25-64, and $125/month for members 65 and older. There is a $100 enrollment fee. Couples receive a 10% per-member discount and families of three or more get 15% off. Annual prepayment waives the general excise tax that applies to monthly billing. The model is pure DPC — no insurance is billed for primary care services. The office is located at 69 Lanihuli Street in Hilo and also offers home visits for members.
